RESUMO

AIMS: Dehydration is one of the common complications of dysphagia and poses significant risks including hospitalization and mortality, but the relationship between dysphagia and dehydration has received little attention. This study aims to determine the prevalence and risk factors for dehydration of patients with dysphagia in eastern China, and to provide reference for early identification and prevention of dehydration. METHODS: A descriptive, cross-sectional design was conducted. Three hundred and thirty-seven (n = 337) patients with dysphagia participated in the study between August and December 2022. Information relating to participants' demographic variables, nutrition, cognition, functional, hydration status and fluid intake was collected. Univariate analysis was used to examine related impact factors, and then binary logistic regression analysis was conducted to determine reliable impact factors. RESULTS: Among 337 patients with dysphagia, the average age was 63.47 ± 16.96, most participants were male (72.1%) and married (91.7%). The prevalence of dehydration was calculated to be 43.9%, the mean plasma osmolality score was 293.53 mmol/L. Diseases with the highest prevalence were stroke (78.3%), followed by hypertension (63.5%). The risk for dehydration increased with older age, usage of more medicines such as diuretics and beta-blockers, worse functional status and lower fluid intake. CONCLUSION: This study found a high percentage of dehydration in patients with dysphagia. Findings can provide a basis for targeted nursing interventions for clinical prevention and treatment of dehydration.

RATIONALE: The standardization, individualization, and rationalization of intensive care and treatment for severe patients have improved. However, the combination of corona virus disease 2019 (COVID-19) and cerebral infarction presents new challenges beyond routine nursing care. PATIENT CONCERNS AND DIAGNOSES: This paper examines the rehabilitation nursing of patients with both COVID-19 and cerebral infarction as an example. It is necessary to develop a nursing plan for COVID-19 patients and implement early rehabilitation nursing for cerebral infarction patients. INTERVENTIONS: Timely rehabilitation nursing intervention is essential to enhance treatment outcomes and promote patient rehabilitation. After 20 days of rehabilitation nursing treatment, patients showed significant improvement in visual analogue scale score, drinking test, and upper and lower limb muscle strength. OUTCOMES: Treatment outcomes for complications, motor function, and daily activities also improved significantly. LESSONS: Critical care and rehabilitation specialist care play a positive role in ensuring patient safety and improving their quality of life by adapting measures to local conditions and the timing of care.

PURPOSE: As hepatocellular carcinoma (HCC) is a complex disease, it is hard to classify HCC with a specific biomarker. This study used data from TCGA to create a genetic signature for predicting the prognosis of HCC patients. METHODS: In a group of HCC patients (n = 424) from TCGA, mRNA profiling was carried out. To recognize gene sets that differed significantly between HCC and normal tissues, an enrichment study of genes was carried out. Cox relative hazard regression models have been used to identify genes that are significantly associated with overall survival. To test the function of a prognostic risk parameter, the following multivariate Cox regression analysis was used. The log-rank test and Kaplan-Meier survival estimates were used to test the significance of risk parameters for predictive prognoses. RESULTS: Eight genes have been identified as having a significant link to overall survival (PAM, NUP155, GOT2, KDELR3, PKM, NSDHL, ENO1, and SRD5A3). The 377 HCC patients were divided into eight-gene signature-based high/low-risk subgroups. The eight-gene signature's prognostic ability was unaffected by a number of factors. CONCLUSION: To predict the survival of patients with HCC, an eight-gene signature associated with cellular glycolysis was then identified. The findings shed light on cellular glycolysis processes and the diagnosis of patients with low HCC prognoses.
